Post-Fracture Recovery in Dogs and Cats
Rebuilding a broken leg after the cast, pin or plate, without losing the muscle to the wait.
A fracture is a broken bone, and once your vet or surgeon has stabilised it, the harder part begins: letting the bone knit while stopping the leg from wasting away. Most dogs and cats walk well again after a fracture. What gets them there is the right amount of movement at the right time, guided by your vet's X-rays, and that timing is what rehab is for.
After the repair, two clocks run at once: the bone needs stillness to heal, and the muscles need gentle, staged use so they don't waste. Rehab keeps both on track, always inside the limits your surgeon sets.

What a fracture is, and what really slows recovery
A fracture is a broken bone. Your vet's job is to line the pieces back up and hold them still; the body's job is to knit them, and that only happens when the broken ends barely move. Bone is fussy about this. It lays down fresh bone across a gap only when movement across that gap stays under roughly two percent. Allow more and you get soft scar tissue instead of bone, which is the whole reason a cast or a plate matters, and why an over-active dog can quietly undo a good repair.
Holding a leg still protects the bone, but it costs you everywhere else. Within about a week of a limb going unused, the muscle around it starts to shrink, the joints stiffen, and the bone itself begins to thin. Vets group these knock-on problems under one name, fracture disease, and heading it off is the main reason rehab exists. A break can look healed on the X-ray while the leg still won't work, because the muscle, the joints and the movement pattern haven't come back. Those are what we rebuild.
Why the repair your vet chose changes the plan
Owners usually arrive holding one sentence: "they put a plate in," or "she's in a cast." That sentence matters more than it sounds, because the method holds the clue to how soon the leg can take weight. A rigid plate heals slowly and quietly, often with no callus you can see on an X-ray. A pin or an external frame frequently lets a leg bear a little weight sooner. None of that is ours to rush: your surgeon's clearance and the follow-up films set every start date, and we shape the rehab around the repair they chose, not the other way round.

How the bone broke shapes the picture too. In dogs, most fractures we see follow a road accident or a fall. In cats, the usual story is a fall from a height, off an HDB ledge, a condo balcony, or an open window, which vets call high-rise syndrome. The back legs take the worst of it, the shin bone most of all, and most of the cats it happens to are under a year old. Window and balcony mesh prevents a great many of them.
Why rehab matters: heading off fracture disease
Bone and muscle both answer to load. Put the right amount through a healing leg and the tissue organises and strengthens; put too little and the bone stays weak and porous; push too much, too soon, and you risk the implant failing or the bone breaking again. Rehab walks that line on purpose, which is why the modern approach is not total cage rest. It is controlled rest: confinement, with short, frequent bouts of prescribed movement layered inside the limits your vet has set.
The complication this prevents is worth naming. After some thigh-bone fractures in young dogs, scar tissue can bind the big front thigh muscle to the healing bone and lock the knee straight, a stiff and hard-to-fix problem called quadriceps contracture. Gentle, early movement of the joint during healing heads it off far more reliably than anything done once it has set in. The wider evidence backs starting early: in one study of dogs after hip surgery, those who began physiotherapy within a couple of days walked markedly better after ten sessions and held on to more muscle than waiting would have allowed.

What recovery actually looks like
Rehab is timed to the way bone heals, in overlapping stages rather than on fixed dates. Yours may run faster or slower; the X-rays lead.
Early protected movement (the first weeks)
While the wound is closing, the work stays gentle: easing pain and swelling, moving the joints by hand so they don't seize up, and light weight-shifting. Strict confinement the rest of the time, and no water work yet.
Controlled weight-bearing (once the surgeon clears it)
After the wound has healed and your surgeon gives the word, often around the three to four week mark, we add short controlled leash work, sit-to-stands, and the underwater treadmill to load the leg safely.
Strengthening (as the bone consolidates)
Now we rebuild the muscle lost to the wait: lower water levels for more load, gentle inclines, more demanding land exercise, and continued hands-on therapy.
Back to normal life
Endurance, and a return to ordinary activity, with any limp or compensation ironed out. Working or sporty animals get a longer reconditioning runway.
The underwater treadmill: loading the leg by the centimetre
This is the part a fracture recovery leans on hardest, and the one tool a general routine can't stand in for. Water height works as a dial for how much weight the legs carry. With the water up at the shoulder, a dog bears only about a third of its body weight; at the elbow, roughly 85 percent; just above the wrist, close to the full amount. Raise or lower the water and you set exactly how much load the healing leg takes, then ease it up week by week as the bone allows.
It does a second job at the same time. A leg that has been splinted or guarded forgets how to move properly, and the treadmill coaxes back a full, slightly exaggerated stride, with more bend through the knee and hock, which rebuilds both muscle and the animal's sense of where the limb is in space. Belt speed, water depth and session length all move with the healing, never ahead of it.
How long fracture recovery takes
The honest answer is that it depends: on the animal, the break, how it was fixed, and how closely the rest is kept at home. What clears each step is not the calendar but your vet's repeat X-rays. As a rough guide to the bone itself healing:

Two outcomes are worth knowing by name. A delayed union is simply slower than expected, and usually still gets there as long as the repair stays stable. A non-union has stalled, and needs another operation to heal. Neither shows on the outside, which is exactly why the follow-up X-rays aren't optional, and why full muscle and function often take several months even after the bone itself has joined.
Protection and pain control
WEEKS 1-4The repair is at its most fragile, so nothing loads it yet. Rehab keeps to pain and swelling control and gentle by-hand movement of the joints above and below the break, so they hold their range while the bone stays still. What opens the next phase is a healed, closed wound and your surgeon's go-ahead.
Early controlled loading
WEEKS 4-8With the wound closed and clearance given, the leg starts taking small, measured amounts of weight. The underwater treadmill carries most of this, holding off the load the bone isn't ready for while the leg relearns an even step. Everything stays short, and inside the confinement your surgeon set.
Progressive strengthening
WEEKS 8-16Once the X-rays show the bone consolidating, load goes up on purpose to rebuild the muscle the still weeks cost. This is the long middle of the recovery, and it runs well past the point the break looks healed on film, because muscle and movement lag bone by weeks. Much the same rebuild carries a leg through post-FHO recovery.
Return to full activity
MONTHS 4+Stamina and a clean, even gait come back last. Ordinary life resumes in stages, each one cleared by how the leg holds up. Working and sporting animals need a longer, graded run-up before full loads, as they would after a cruciate repair. How soon depends on the break and the repair, not the calendar.
Getting the rest right at home
The repair holds the bone; your confinement is what lets it knit. That part happens at home, and it is where recoveries are quietly won or lost.
Cage rest, but not bubble wrap
"Crate rest" is the instruction almost every owner is handed, and it is right, but it is only half the picture. Total stillness with no movement at all lets the muscle waste and the joints stiffen, the very fracture disease we are trying to avoid. What the leg actually needs is controlled rest: strict confinement between sessions, plus the small, specific movements your rehab team sets. That doesn't contradict your surgeon's confinement order; it lives inside it.
The two ways it usually goes wrong
Recoveries tend to stumble at one of two opposite poles, and both come from good intentions. One is doing too much: letting the pet off confinement early because it "seems fine," allowing a jump onto the sofa or a dash up the stairs, slipping the cone off "just for a bit." That is a real cause of implants loosening and bones failing to knit. The other is doing too little: total cage rest with no prescribed movement, which wastes muscle and stiffens joints. Around those sit the everyday slips, stopping pain relief early, or reaching for human painkillers, and on that last point a firm warning: paracetamol and ibuprofen are poisonous to pets, cats especially. The middle path, set by your vet and rehab team, is the one that gets a leg working again.
When to call your vet
Some changes during a fracture recovery can wait for the next check; a few really can't. When you are not sure, film a short video and call your vet.
Call your vet today
- A leg that was improving suddenly won't take any weight at all.
- The limb looks newly crooked or twisted, or there's a new lump or click.
- Heat, marked swelling, redness, or any discharge (pus, blood, a bad smell) at the wound or around the frame pins.
- The wound opens, or the stitches pull apart.
- Fever, flatness, or going off food after the first few days.
- A cast or frame that has slipped, got wet or soiled, or is making the toes below it swell.

- Progress that has flattened out and stopped improving.
- A mild limp that comes and goes.
- A little clear weeping at a frame pin can be normal; a change in its colour, smell or amount, or new soreness, is not.
A word for cat owners: cats hide pain unusually well. A cat that has stopped jumping to its usual perches, is hiding more, grooming one spot raw, or eating less is telling you something, even without a single cry.

- How was the break fixed: plate and screws, a pin, an external frame, a cast, or managed without surgery?
- How strict is the confinement, and for exactly how long?
- Which movements are off-limits right now, and which are allowed?
- When is the next X-ray, and what does it need to show to move up a level?
- When can rehab start, and when specifically can hydrotherapy or the underwater treadmill begin?
- What should the wound and any pin sites look like normally, and what would count as a worry?
- Is the implant meant to come out later?
- Anything about my pet's age or breed that changes the healing outlook?
The outlook, and your vet's lead
Most dogs and cats come through a fracture well. When the repair holds and rehab runs alongside the healing, the great majority get back to comfortable, near-normal movement, and many to full athletic life. A few carry a little stiffness or a faint limp, more likely after a joint-surface break or a long stretch of disuse, and that is usually something rehab can keep improving.

Common questions about fracture recovery
When can my dog or cat start rehab after a fracture?
Gentle rehab often starts within days of the repair, while the wound is still healing, to keep the joints moving and hold off muscle loss. Water work on the underwater treadmill waits until the wound has closed and your surgeon clears it, commonly around three to four weeks. Your vet's X-rays set the pace throughout.
Will my pet's leg ever be completely normal again?
Most pets regain comfortable, near-normal use of the leg. How close to "as before" depends on the break, how it was fixed, and how well the rest and rehab are followed. Some animals keep a slight stiffness, especially after a joint-surface fracture, and rehab can usually keep chipping away at that.
How long does the crate rest need to last?
Confinement commonly runs somewhere between four and eight weeks, but the exact length is your surgeon's call, based on the break and the repeat X-rays. Remember it should be controlled rest, strict confinement plus the small prescribed movements we set, not total stillness, which wastes the leg.
Is hydrotherapy safe while there's still a plate or pins in the leg?
Yes, once the wound has fully closed and your surgeon has cleared it. The water carries part of the body weight, so the leg moves and rebuilds muscle under far less load than walking on land, which makes it one of the gentler ways to start using a repaired limb.
My cat fell from a window. What should I do?
See your vet first for a full check and X-rays, since falls from a height often bruise the chest or jaw as well as break a leg. Once the fracture is treated and your vet gives the go-ahead, rehab helps rebuild the leg. Fitting mesh to windows and balconies prevents most repeat falls.
